Unsafe Bathroom
An employee in an office building in Newton, NJ found a small puddle on the floor in the restroom. After reporting it to her manager, he contacted the maintenance supervisor to check it out. It didn't seem like a big deal, so they mopped the floor and put a fan on it to dry it up.
The next day, the water was back and they decided to investigate further. As it turned out, there was a small pipe in the wall which had been dripping.
